View and Print Online
1. Click on the application name
2. When the application is on your screen, you will see a menu of commands along the top. Select File and then from that menu select Print.
3. Use the back arrow on the browser window to return to this page.
Download Application to Your Computer
1. To download the application, right-click on the application name.
2. Choose Save Link As (Netscape) or Save Target As (Internet Explorer) to save the file to your desktop (Select Desktop in the box marked Save in).
3. Double-click on the new icon on your desktop when the file has finished downloading.

1. Review and/or complete the application. Please make sure that all questions have been answered. If the question does not apply to your particular situation, please indicate by answering either "N/A" or "No".
