1. What is my policy number?
Your policy number is located on the top right corner of your policy Declarations page. Your policy number will also be located on Certificates of Insurance that have been issued on your behalf.

2. What are the limits of liability of my policy?
The Declarations page of your policy will list your primary limits and indicate if higher limits of coverage have been issued. If you have limits higher than $1,000,000 each loss, they will be listed on the Higher Limits Endorsement attached to your policy.

3. Can you send verification of my insurance (or a Certificate of Insurance) to a hospital or other entity?
Yes. Certificates of Insurance can be requested through our online Customer Service Center. Or you can contact Carol Sigler in our Underwriting Department at (404) 842-5600 or 800-282-4882. She can also be contacted via email at csigler@magmutual.com.

4. How do I receive information on my payment plan or other accounting information?
Premium payment information may be acquired by contacting Accounts Receivable at (404) 842-5600 or 1-800-282-4882, or via email at AcctsReceivable@magmutual.com.

5. How do I report a medical malpractice claim?
It is very important that you report to us promptly if you have received notice from a patient or an attorney regarding a potential claim. You may report a claim or incident to us by telephone by contacting Leanne Villanueva at (404) 842-5600 or 1-800-282-4882 or in writing to our mailing address. If Ms. Villanueva is unavailable, you may speak with the operator and advise her you need to report a claim. The operator will direct the call to a member of our claims staff. 6. In what states can MAG Mutual Insurance Company provide coverage?
MAG Mutual is licensed to provide insurance products and services throughout the Southeast. We meet the insurance needs of the medical community in Alabama, Florida, Georgia, North Carolina, South Carolina, Tennessee, and Virginia.

7. How many days of Locum Tenens coverage does my policy provide?
Locum Tenens coverage is designed to provide coverage for substitute physicians working in your place on a temporary basis due to your vacation, illness or other absence from your usual practice, but only if your contract with the temporary substitute physician specifies that you will provide insurance coverage.

Your medical malpractice policy with MAG Mutual provides automatic shared-limit coverage for up to 60 days for temporary substitute locum tenens physicians. If your specific needs require more than 60 days, please contact one of our customer service representatives for additional coverage.

8. What is the cost for a reporting endorsement or "tail" coverage?
The cost for an extended reporting endorsement or "tail" coverage is dependent upon such factors as the amount of time you have been insured on your current policy, your medical specialty and liability limits. The premium typically can range anywhere between 1.5 to 3 times your current policy premium.

9. How can I provide coverage for my professional employees?
Many of your professional employees automatically share your limits of coverage. MAG Mutual provides optional coverage for other employees on a shared- or separate-limit basis.

If you wish coverage for any of the following employees, you must contact a customer service representative and have coverage specifically added to your policy.

Employees not automatically insured include:

  • Physician Assistants
  • Employees who administer anesthesia
  • Nurse Midwives
  • Podiatrists
  • Chiropractors
  • Dentists
  • Psychologists
  • Osteopaths
  • Medical Doctors
