Georgia PDMP Registration – by provider’s date of birth

As you know from our prior email, all Georgia prescribers are now required to register with the Georgia Prescription Drug Monitoring Program (PDMP) by January 1, 2018.  Due to the large number of providers who must register by that date, the Georgia Department of Public Health is encouraging registration by date of birth. 

 

If you were born in:                                        Register for the PDMP in:

 

January, February, March                               July 2017

 

April, May, June                                               August 2017

 

July, August, September                                 September 2017

 

October, November, December                    October 2017

 

To ensure that all prescribers and dispensers are registered by January 1, 2018, the two delegates (staff without a DEA number) that are allowed under the new law will register after January 1, 2018. 

 

To register, please click here.  You will need to provide your:

 

  • Name and business address
  • Primary phone number
  • Last 4 digits of social security number
  • DEA number
  • NPI number
  • Professional license number and type
  • Health care specialty
